  As a reminder, it is not appropriate to call a doctor to your home when:

• the child complains of pain in any location (head, abdomen, ear, etc.);

• the child is vomiting and has diarrhoea;

• feel chest tightness or suffocation;

• when you notice rashes on your skin;

• when a child has been poisoned by any substance and has started to behave strangely, unusually;

• In cases of trauma, burns, frostbite and bleeding.

In all these cases, either urgent care or further tests and specialist advice are needed. When you arrive at the clinic, even if your doctor is not working at the time, you will be given the care you need.

Important!

• Please remember that children under 16 years of age must be accompanied by a parent or guardian when attending a clinic for an illness, vaccination or preventive check-up.

• When a doctor is called to your home, the child's parents or guardians must be present.

• When you call the doctor at home, don't forget to tell the receptionist the door code (if you live in an apartment block).

Information for parents!

• Infants under 1 year of age are screened once a month.

• Children under 3 years of age are screened once every six months. Children up to 6 years old are screened once a year.

• Year 7 - Preventive health check-up before school.

• Students aged 7 to 18 years are tested once per calendar year.

• Children aged 1 year and older are examined once a year by a dentist. Children aged 6 years and older have their first molars covered with silane, an anti-cavity tooth decay agent.

• A preventive check-up assesses the child's growth and development, his or her health, carries out the necessary tests, and refers the child for specialist consultations.

Sanatorium treatment

Patients are referred for sanatorium treatment in accordance with the Order No.V-50 of the Ministry of Health of the Republic of Lithuania of 17 January 2008 "On the organisation of medical rehabilitation and sanatorium (anti-recidivism) treatment". The Order regulates the procedure for referring patients for sanatorium treatment and defines the special requirements for the provision of medical rehabilitation (diseases for which patients can be referred for treatment).

Patients can be referred for sanatorium treatment from inpatient and outpatient health care facilities (outpatient clinics, polyclinics ). In the acute phase of the illness, the patient is given the first stage of rehabilitation. These services are provided by the specialists of the Kėdainiai Hospital. If the functional impairment persists, patients may be referred for outpatient rehabilitation or sanatorium treatment in an inpatient hospital (sanatorium). Adult patients can only be referred for outpatient rehabilitation from the outpatient clinic, while children up to the age of 18 can be referred for health restoration treatment and outpatient rehabilitation. The treating doctor is responsible for the validity of the referral.
When patients are sent for sanatorium treatment, an extract from the medical records and a certificate for a travel voucher must be completed. A representative of the Territorial Patients' Fund working at the VšĮ Kėdainiai PSPC issues a certificate confirming the right to compensation for medical rehabilitation.

Children under 8 and young patients who have been assessed by the Invalidity and Disability Determination Service as having a special need for permanent care or nursing have the right to be accompanied by a carer (adult) when going to medical rehabilitation facilities.

Insured persons receive only one course of sanatorium treatment per calendar year.

MEDICAL CHECK-UP PROCEDURES FOR DRIVERS

A new procedure for medical examinations of drivers approved by Order No 301 of 31 May 2000 of the Minister of Health of the Republic of Lithuania "On the amendment of the procedure for preventive medical examinations in health care institutions". The medical certificate E083-1 for driver's health check-up may be issued to persons not more than one year younger than the legal age for driving a vehicle of that category.

Drivers and driver candidates registered with the Kėdainiai PSPC must be examined by a family doctor (children under 18 - paediatrician) within one year. Children under 18 years of age are only registered for a child and adolescent psychiatric consultation at the Drivers' Commission during the Tuesday opening hours of the Drivers' Commission.

Drivers who are registered with other medical institutions but wish to have their health checked at the Kėdainiai PSPC must have a medical certificate/referral (form 027/a) or a medical certificate/referral letter (form 027/e) from a family/children's doctor.

Drivers who have been registered with other municipal health services in the last five years must have a "Medical Records Extract/Referral" (form 027/a) completed by the psychiatrist at the mental health centre that provided the care.

When to call a doctor to your home?

Doctor calls to the home are recorded when:

• the patient has a fever of 39 degrees or more;
• the patient does not walk;
• have flu-like symptoms (high temperature, chills, bone and muscle pain) during an influenza epidemic.

 The complaining patient is registered on the same day as an emergency:

• sudden onset, severe pain in any location (head, heart, abdomen, etc);
• shortness of breath, a feeling of heaviness in the chest;
• bleeding;
• sudden impairment of vision, speech, mobility;
• in the event of an injury.

 IMPORTANT!

• Don't forget your personal documents when you arrive at the clinic. You need a passport for working people, a disability certificate for disabled people and a pensioner's certificate for pensioners.
• Patients are registered in advance for a scheduled visit (prescription).
• The doctor sees patients at the times indicated on the vouchers. Minor deviations may occur in relation to emergency care for patients brought in by ambulance.

Patient outpatient card

The document of a healthcare institution, which is used to settle accounts with the Territorial Health Insurance Fund, provide the necessary information, and carry out audit checks at the request of law enforcement authorities. In the case of consultations with other medical institutions, the outpatient card is not given to the patient, as all the necessary key information is recorded by the doctor making the referral in the 'Extract from medical records' form No 027. The patient can copy the information he/she wants from the card at his/her own expense if necessary. In the event of a transfer to another medical institution, the outpatient card shall be forwarded to that medical institution after the necessary paperwork has been completed.

For the attention of pregnant women

There is a antenatal care room No.55 on the 2nd floor of the Kėdainiai PSPC, open daily from 8.00 to 16.00. On arrival at the clinic, you can register the same day at the adult reception at window 2. The clinic offers outpatient antenatal care by a midwife and schedules follow-up appointments.

Changes to the procedure for medical rehabilitation services

From 1 January 2023 , referrals for medical rehabilitation services will be made by a physical medicine and rehabilitation doctor instead of a family doctor.

As a result, from 1 January 2023, GPs will not be able to write referrals for medical rehabilitation services.

From 1 January 2023, the first phase of medical rehabilitation services will be known as initial medical rehabilitation services.

The family doctor writes a referral only for initial medical rehabilitation services.

Initial outpatient medical rehabilitation services for the same illness are paid for from the budget of the Compulsory Health Insurance Fund (hereinafter - "CHIF") no more than 2 times a year.

Until 31 December 2022, a referral for medical rehabilitation services signed by a family doctor and the form No 2011T "Notification of consent to reimbursement of rehabilitation costs from the budget of the Compulsory Health Insurance Fund" issued by the Territorial Health Insurance Fund will remain valid in 2023, until the expiry of the period of validity of these documents.

The dental unit has 11 dentists and 10 dental assistants. Therapeutic (dental treatment) and surgical services (teeth straightening, abscess opening, etc.) are provided.

The department has 2 dental hygienists who provide professional oral hygiene. There is a charge for these services for patients over 18 years of age. Patient registration takes place at the reception of the dental unit.

Children's tooth decay prevention programme

Since 2005, the Dental Department of the Kėdainiai Primary Health Care Centre has been running a children's tooth decay prevention programme. The aim of the programme is to protect the newly erupted permanent molars from early caries. To this end, the furrows and hollows of the permanent molars are coated with silane, a material that protects the chewing surfaces of the teeth from bacteria and carbohydrates.

Children's permanent molars are coated with silantic materials between the ages of 6 and 8, and permanent second molars between the ages of 11 and 13. If there is a high risk of tooth decay (as determined by the dentist), silanes are also used to cover the furrows and pits of other teeth, such as the capillaries and sometimes the upper lateral incisors. It takes between 5 and 6 minutes per tooth to apply the silane. The procedure is completely painless and does not require the use of a drill. The grooves and pits are thoroughly cleaned to remove any plaque, rinsed, dried and covered with a liquid substance which is hardened with a light (helium) lamp. The silants remain in the sockets for several years and protect the tooth from external influences. In addition, the silants contain fluoride, which is then released and helps to strengthen the enamel of the furrow or socket. Therefore, silants are an excellent preventive measure.

If you wish to make use of this option, you can contact the reception of the Dental Department between 7.00 and 16.00 every working day. Children must be accompanied by a parent/guardian.

What should you do if you have a tooth extracted?

• Keep the swab in your mouth for 30 minutes after the extraction. If the bleeding persists, keep the new swab in place for another 20 minutes.
• Do not rinse for the first day after tooth extraction to avoid rinsing away the clot. In the following days, you can rinse with disinfectant solutions (especially after eating).
• To reduce swelling, place an ice bladder on the cheek and hold for 30 minutes.
• Don't starve, eat softer and gentler foods.
• Brush your teeth gently, morning and night.
• Don't overdo it.
• Avoid alcohol for the first 24 hours.

The Mental Health Unit at the Kėdainiai Primary Health Care Centre was established on 1 April 1999.

The Mental Health Centre has a team of specialists:

• psychiatrists;

• child and adolescent psychiatrist;

• psychologists;

• social workers;

• Mental health nurses.

These specialists advise on:

• children and adults;

• people suffering from addiction diseases;

• organises occupational therapy for mentally ill people;

• psychological diagnosis;

• carry out preventive health checks;

Consultation

Psychologists provide counselling with a referral from a doctor, or you can come on your own. Psychodiagnostic tests, counselling on parent-child relationships, psychological support in cases of bereavement, violence, other psychological crises and behavioural disorders in children and adolescents. In case of crises, you should come to the medical psychologist's office or call (during office hours).

The child and adolescent psychiatrist consults patients up to the age of 18 in Room 12 (IIa) of the Mental Health Unit. Diagnoses illnesses, prescribes and monitors the effectiveness of medical treatment, and monitors patients dispensed for mental illness. Registration with this specialist is required at the children's reception.

Social workers provide counselling on social problems (information, counselling, mediation, representation of patients in other institutions, organisation of occupational therapy for mentally disabled patients; organisation and work with self-help groups). Anonymous rapid tests for chronic infectious diseases: HIV, Hepatitis B and C. Works in harm reduction programmes: substitute supportive care, 'low threshold' office services. These professionals work in the Mental Health Unit's Social Worker Room 1, tel. 8 695 18681. They can be accessed without a referral.

Mental health and care

Everyone has experienced brief moments of confusion, anxiety, sadness or even despair. If these experiences have a clear cause, if they are of short duration, they should not be considered as a morbid condition. However, if they persist for weeks or months, they are disruptive to normal behaviour and life and help should be sought. The more pronounced and severe the disorder, the harder it is to persuade a person to seek treatment. Doctors and family members of some patients know how much effort it takes to get someone to accept the help they need...

What is mental health? Mental disorders affect thinking, behaviour, feelings and relationships. Like many physical illnesses, the causes of mental disorders are linked to both the person's psychological experiences and the environment around them. The sufferer sees his or her experiences, feelings and emotions as more real than the reality in which we all live: he or she perceives his or her surroundings in a different way, understands what is being said to him or her in a different way, and sometimes he or she hears, sees or smells things that are not there, and no one is able to convince him or her that he or she is wrong.

Mental health: it is not just the absence of mental illness or disorder; it is the ability to be yourself among others; it is an emotional and spiritual state that allows you to experience joy and endure pain, frustration and sadness; it is the ability to express yourself in a way that is pleasurable for yourself and for those around you; it is the ability to make your own choices and be accountable for them.

Mental health care is specialised health care that provides psychiatric care (diagnosis and treatment of mental disorders) and helps patients adjust to life in society.

• coercion in society;
• family crisis;
• non-compliance with work and rest rules;
• Alcohol and drug use;
• spreading aggression and violence in the media, etc.

Internal Medical Audit Members/Experts Group

Since 20 September 2022, the internal medical audit team of members/experts has been working at the Kėdainiai PSPC, the composition of which has been approved by the Director's Order No 112 of 21 September 2022 "On Approval of the Composition of the Members/Experts of the Internal Medical Audit Team".

Internal Medical Audit/Expert Group:

• participates in the definition of the quality policy, objectives and quality indicators for the healthcare services provided by the Kėdainiai PSPC;
• conducting planned and unplanned internal medical audits;
• organising the recording of adverse events and non-conformities, analysing their causes and proposing preventive measures to the head of the establishment;
• collecting and summarising information on patients' satisfaction with the services they receive;
• examines patient complaints within its remit and submits the findings of its analysis and proposals to the head of the institution;
• contributing to the preparation of the institution's documents relating to quality assurance activities in healthcare;

Treatment Council

Kėdainiai PSPC Treatment Council:

• discusses the organisation and improvement of personal health care;
• periodic clinical conferences;
• considering the acquisition of new personal healthcare technologies;
• may make proposals of a recommendatory nature to the head of the institution on matters under consideration

Nursing Council

Approved by Order No 114 of 6 October 2022 of the Director of the Public Institution Kėdainiai PSPC.

Nursing Council of Kėdainiai PSPC:

• discussing how to organise and improve patient care;
• may make proposals of a recommendatory nature to the head of the institution on matters under consideration. If the head of the institution does not agree with the proposal, the nursing council may submit its proposal to the shareholder and/or the authority exercising the rights and obligations of the shareholder/owner of the public institution.

The Secretary of the Nursing Council shall be elected at each meeting.

Board of Supervisors

Supervisory Board of the Public Institution Kėdainiai PSPC:

• analyse the institution's performance;
• hear and evaluate the body's annual activity report;
• coordinate draft decisions prepared by the institution
• (increasing or decreasing the range of services provided, changes in internal structure, staff remuneration, etc.)

Psychiatric day hospital

Since 17 December 2013, the Mental Day Hospital has been providing services in Kėdainiai PSPC.With the Mental Day Hospital in Kėdainiai, there is no longer a need for patients to be referred to psychiatric hospitals, where they receive adequate support and treatment, but are separated from their families and loved ones. The day hospital focuses on rehabilitation and employment activities for patients. Upon arrival, patients are encouraged to maintain their independence, daily living habits, psychological communication, household and work skills. Patients are treated in the day hospital for a maximum of 30 working days. Most importantly, unlike in conventional hospitals, patients arrive early in the morning and can return home in the evening after spending the day. This gives them the opportunity to recover in the comfort of their own family, without having to leave their usual environment, and it eases the daily routine of the family members and relatives who care for them, giving them respite and the opportunity to carry out their normal work.

Each inpatient receives an individual treatment programme, with appropriate medication, psychotherapy sessions, relaxation sessions and psychoeducational programmes for both patients and their families, depending on their health status.

Psychic Days inpatient services are provided by specialists team:

• psychotherapist - Aušra Račkauskienė;
• medical psychologists - Simona Kutavičė;
• Asta Šmigelskė, Senior Nursing Specialist;
• mental health nurse - Zina Lapinskienė
• occupational therapist - Kristina Bugenienė;
• physiotherapist - Kamilė Augienė;
• social workers - Lina Dubauskienė, Edita Navickienė, Inga Vilčinskienė;
• art therapist - Vilija Racevičienė.

Treating in a day hospital:

• reducing the time spent in hospital (lowering costs);
• Patients continue to receive adequate treatment and rehabilitation until outpatient treatment is available;
• the patient does not lose contact with the family;
• Avoids the desocialisation of the patient that occurs with prolonged psychiatric hospitalisation;
• After discharge from inpatient to outpatient treatment, handover between the psychiatrist and the patient's family doctor is ensured.

Mental health day hospital services:

• individual consultations with a psychiatrist/psychotherapist (assessment of mental state or behaviour);
• individual psychotherapy sessions;
• individual and group consultations with medical psychologists;
• individual counselling by a mental health nurse (counselling or training on prescribed or self-selected medication);
• individual counselling by a social worker (counselling and training related to individual care and daily activities);
• Individual occupational therapy (memory, orientation, perception or attention skills);
• physiotherapy services (movement skills training);
• Relaxation (psychological skills training);
• Light therapy;
• group psychoeducational sessions;
• group art therapy.
