    01. Become our patient

    DESCRIPTION OF THE PROCEDURE FOR ENROLMENT AT THE PUBLIC INSTITUTION KĖDAINIAI PRIMARY HEALTH CARE CENTRE

    PACIENTŲ PRISIRAŠYMAS PRIE VšĮ KĖDAINIŲ PIRMINĖS SVEIKATOS PRIEŽIŪROS CENTRO, ŠEIMOS GYDYTOJO PASIRINKIMAS 

    Before choosing a doctor, the person shall find out by telephone, e-mail or by visiting the establishment whether the workload of the doctor to be chosen allows the doctor to be chosen in accordance with the procedure laid down in this Order.

    You can fill in an application form to be admitted for treatment at the VšĮ Kėdainiai Primary Health Care Centre or Mental Health Centre:

    • On arrival at the outpatient clinic of the Kėdainiai Primary Health Care Centre;

    • send the form to the email address provided by the Centre at [email protected]. (form here...) 025-025-1/a request, which must be signed with an electronic qualified signature;

    • via the Public Information Resources Interoperability Platform (Electronic Government Gateway) or by any other electronic means that allows for the authentication of a person's identity, using an electronic interactive enrolment service;

    • By post or courier, accompanied by a copy of your identity document;

    • In the event that the person is unable to submit the application form No. 025-025-1/a in the above-mentioned ways due to an emergency or quarantine in the country or municipality where the person resides, the person shall complete and submit the application form No. 025-025-1/a in the About Us - Become Our Patient section on the website of the Centre;

    • The original of the Form 025-025-1/a must be submitted to the Centre (by direct arrival, post or courier) within 30 calendar days (but not before the end of the emergency or quarantine).

    • Apply at (form available here...) to be treated by another primary care doctor of the person's choice, in the order indicated above.

    1.The person (guardian) who wishes to join the Public Institution Kėdainiai Primary Health Care Centre (hereinafter - the Centre):

    • Contact the statistics office (Department of Family Medicine, 2nd floor, room 35, Budrio str. 5, Kėdainiai);

    • Provide proof of identity (ID card, passport, birth certificate for children under 18);

    • Filling in the "Application for treatment at the primary healthcare institution of your choice" (form 025-025-1/a):

    • In the relevant sections of the application form, the person indicates his/her name, surname, residential address, the name of the Centre, the name of the doctor chosen, the date on which he/she completed the application form, and signs the application. Part II is completed, in which the name of the mental health centre, the date of completion of the application and the signature are entered.

    • You pay a fee of EUR 0.29 at the Centre's cash desk, located next to reception. If the change of the PHC facility is made before 6 months, the resident pays a fee of €2.90.

    2.To make an appointment with a doctor working in an outpatient clinic, if you wish person:

    • A person (carer) who wants to see a doctor working in an outpatient clinic should contact the general nurse in that clinic.

    • On presentation of proof of identity (identity card, passport, birth certificate for children under 18 years of age) and payment of a fee of EUR 0.29, he/she fills in application form 025-025-1/a.

    • Provide proof of identity (ID card, passport, birth certificate for children under 18);

    • The application form should be completed in the order indicated above.

    3. Person (guardian)who wishes to sign in with another Centre a doctor working at the Centre:

    • Contact the Centre's statistician (Department of Family Medicine, 2nd floor, room 35, Budrio str. 5, Kėdainiai)

    • Provide proof of identity;

    • Fill in form 025-025-2/a "Request for authorisation to be treated by another doctor of the chosen primary outpatient health care facility or mental health centre".

    • The application form is filled in with the name of the doctor of your choice and signed.

    4. When a doctor changes specialty, after termination of employment:

    • The Centre's administration shall arrange for the service of persons registered with the Centre. The Centre's administration shall appoint another doctor(s) if the person does not choose another doctor or another PAASP. The appointment of another doctor and the re-registration of persons shall be by order of the Director of the Centre. The patient shall be informed in writing of the change of doctor. If the patient agrees to subscribe to and see another doctor or to choose another doctor, he/she shall fill in the application form No 025-025-2/a.

    • If the patient agrees to see the assigned doctor or chooses a different doctor, he/she completes the Request for treatment with another doctor of the chosen primary care institution (form 025-025-2/a).

    02. Free services

    FREE PERSONAL HEALTHCARE SERVICES PROVIDED BY THE PUBLIC INSTITUTION KĖDAINIAI PRIMARY HEALTH CARE CENTRE

     

    The following free outpatient personal healthcare services are provided:

    1.The right to free personal health care services paid by the State Patients' Fund (according to the agreement with Kaunas Territorial Patients' Fund) from the funds of the PSDF, provided by the VšĮ Kėdainiai PSPC, shall be granted to the persons insured with compulsory health insurance and residents of the VšĮ Kėdainių PSPC who have signed up to receive primary personal health care services.

    2.Nomenclature of personal health care services provided free of charge by the Public Institution Kėdainiai PSPC:

    2.1.Services of professionally qualified doctors (family doctor, internal medicine doctor, surgeon, obstetrician, gynaecologist, paediatrician), general nurse, midwife, case manager) providing primary outpatient personal health care:

    2.1.1. diagnosing and treating diseases within the scope of competence;

    2.1.2. ensuring continuity of treatment;

    2.1.3. issuing prescriptions for medicines and medical devices;

    2.1.4. making referrals for consultation and treatment to a specialist in the relevant field, in cases that are not within the competence of the family doctor;

    2.1.5. carrying out preventive health checks;

    2.1.6. vaccination of children and adults;

    2.1.7. developmental monitoring of newborns, infants and children;

    2.1.8. monitoring of pregnant women;

    2.1.9. the examination of patients' temporary incapacity for work, in accordance with the legislation governing it;

    2.1.10. issuing certificates for sick leave, pregnancy and maternity leave;

    2.1.11. completing referrals to the Agency for Protection of Rights of Persons with Disabilities under the Ministry of Social Security and Labour of the Republic of Lithuania;

    2.1.12. issuing death certificates if the cause of death is certain.

    2.1.13. Incentive services provided by doctors, nurses and midwives in primary outpatient care;

    2.2 Primary and secondary dental personal care services (dentist, dental assistant, oral hygienist) ;

    2.3 Home care services (general nurse, nursing assistant, physiotherapist);

    2.4 Diabetes nursing (diabetes nurse) services;

    2.5. primary outpatient mental health services (psychiatrist, child and adolescent psychiatrist, psychotherapist, medical psychologist, social worker, mental health nurse, case manager);

    2.6. Psychiatric day hospital services (psychiatrist, psychotherapist, medical psychologist, social worker, mental health nurse, case manager, occupational therapist, physiotherapist, physiotherapist, art therapist);

    2.7. Free prevention programme services:

    2.7.1 Early diagnosis programmes for cervical cancer ;

    2.7.2 Early diagnosis programmes for prostate cancer ;

    2.7.3 Early breast cancer screening programmes;

    2.7.4 Screening and prevention programmes for people at high risk of cardiovascular disease ;

    2.7.5 Early diagnosis programmes for colorectal malignancy ;

    2.7.6. programmes for the application of enamelling materials to children's molars.

    3. Emergency medical care.

    The list of essential medical assistance, i.e. diseases and pathological conditions for which medical assistance is provided free of charge, is approved by the Order of the Minister of Health of the Republic of Lithuania No.V-208 of April 8, 2004 "On the Approval of the Procedures and Scope of Provision of Essential Medical Assistance and Services of Essential Medical Assistance", and its amendments and additions.

V-943 „Dėl pirminės ambulatorinės asmens sveikatos priežiūros paslaugų organizavimo ir apmokėjimo tvarkos aprašo bei pirminės ambulatorinės asmens sveikatos priežiūros paslaugų ir bazinių kainų sąrašo tvirtinimo“ ir Lietuvos Respublikos Sveikatos apsaugos ministro 2005 m. gruodžio 22 d. įsakymo Nr. V-1013 „Dėl Lietuvos medicinos normos MN 14:2005 „Šeimos gydytojas. The scope and procedure of other primary outpatient personal health care physicians and the services provided are specified in the medical norms approved by the Ministry of Health of the Republic of Lithuania and in the Orders of the Minister of Health of the Republic of Lithuania.

    A family doctor can only prescribe tests that are within his/her competence. In each case, the family doctor assesses the patient's state of health and decides on the tests within his or her competence.

    If necessary, a general blood test, biochemical blood tests such as lipidogram, potassium, sodium, creatinine, alanine aminotransferase, aspartic aminotransferase, alkaline phosphatase, bilirubin, glucose, uric acid, are carried out free of charge, on the prescription of a family doctor. He may also prescribe, if necessary, a urine dipstick test and a urine albumin/creatinine ratio test; tests for indicators of inflammation, i.e. Inflammation tests, such as C-reactive protein (CRP), erythrocyte sedimentation rate (ESR) and thyroid function tests (TTH); glucose tolerance test; coprogram; gynaecological vaginal and cervical smear; enterobioassay; electrocardiography; spirometry to assess peak expiratory flow rate (PEF); forced expiratory volume in the first second (FEV1) and forced vital capacity (FVC) values; pulse oximetry; investigations for (and evaluation of the results of) COVID-19 disease; and natriuretic peptide test.

    In some cases, your GP may prescribe:

    • glycolysed haemoglobin test;
    • blood clotting tests (prothrombin time, international normalised ratio, activated partial thromboplastin time);
    • prostate-specific antigen testing (also for patients after radical treatment for prostate cancer);
    • blood group and Rh factor tests;
    • syphilis (RPR) and specific (treponemal) testing with T. pallidum antigen (TPHA) testing;
    • testing for viral hepatitis B and C;
    • urine crop;
    • human immunodeficiency virus test;
    • Rapid test for group A beta haemolytic streptococcus;
    • a test for fecal occult haemorrhage;
    • a tuberculin sample;
    • oncocytology cervical smear;
    • administering and administering smallpox and monkeypox vaccinations to persons at risk.

    The family doctor also prescribes tests for healthy patients in certain age groups under the following prevention programmes: cervical, breast, colorectal, prostate cancer and cardiovascular diseases.

    05. Programmes financed by the Compulsory Health Insurance Fund budget

    We invite you to make better use of the opportunities offered by the state to get free health checks

    You are invited to get tested through preventive health programmes, which allow you to detect abnormalities early, identify serious diseases and treat them effectively. Diseases that are detected early are much easier to treat than those detected at later stages. Preventive programmes are free of charge for patients (financed by the PSDF budget).

    Prevention programmes are aimed at patients of a certain age. However, if a person feels unwell but does not fall into the age group, he or she should under no circumstances delay in contacting his or her family doctor. He or she will carry out the necessary tests and, if necessary, issue a referral for a consultation with a specialist.

    Programme for the financing of measures to prevent cervical malignancies

    • The aim of the programme is to reduce the incidence of cervical malignancies and mortality among Lithuanian women.
    • Women aged 25-34 years have a cytology smear test once every 3 years and women aged 35-59 years once every 5 years . The results may be followed by additional tests to objectively confirm or refute the diagnosis.

    Funding programme for screening mammography for breast cancer

    • The aim of the programme is to reduce the mortality rate of Lithuanian women from breast malignancies.
    • Moterims nuo 45 iki 74 metų (imtinai) kartą per 2 metus atliekamas mamografinis tyrimas.
    • If you have a referral from your family doctor, you can register in advance at the reception desk of the Consultation Clinic of the Kėdainiai Hospital (or any other health care institution providing mammography services).
    • The patient is informed of the results of the test by her family doctor.

    Funding programme for early diagnosis of prostate cancer

    • The aim of the programme is to improve the diagnosis of early-stage prostate cancer and to apply radical treatment methods for prostate cancer in order to reduce disability and mortality.
    • Men aged 50 to 69 years (inclusive) and men aged 45 years and over who have had a parent or brother with prostate cancer have a blood test to determine the level of prostate-specific antigen (PSA) in their blood.
    • If your PSA level is above the normal range, your GP will refer you for a urology consultation.
    • Further tests will determine the diagnosis.

    Funding programme for early diagnosis of colorectal cancer

    • The aim of the programme is to improve the detection of early stages of colorectal cancer and to reduce mortality from the disease.
    • For people aged 50 to 74 years, a covert bleeding test is carried outonce every 2 years. If the test is negative, the patient is healthy; if the test is positive, the family doctor will refer the patient to a specialist for a colonoscopy or other tests.
    • These tests will help to confirm or rule out a diagnosis of colorectal cancer.

    People at high risk of cardiovascular disease, Programme for the financing of screening and prevention measures

    • The aim of the programme is to reduce avoidable mortality from cardiovascular disease through prevention and treatment by identifying patients at risk of cardiovascular disease (hereafter referred to as 'at risk group'), organising their preventive care, and making lifestyle changes and, if necessary, prescribing the necessary treatment.
    • Men and women aged 40 to 60 years (inclusive) should consult their GP, who will carry out the necessary tests and assess the patient's risk of cardiovascular disease.
    • How often should I be tested? If assessed by your GP:
    • very high risk - after 1 year;
    • high risk - after 2 years;
    • low and medium risk - after 4 years.
    •  If the family doctor determines that the likelihood of cardiovascular disease is high, he or she sends the patient to specialised centres for a more detailed examination. If necessary, treatment is given.

    06. Nursing services in the patient's home

     

    Nursing services are provided by the Home Nursing Service

     

    • The Kėdainiai Primary Health Care Centre provides outpatient home nursing services. These are personal health care services provided in patients' homes to ensure the availability and continuity of nursing services, to meet the patient's nursing needs at home, to promote self-care and to improve the patient's quality of life.

    • Outpatient nursing services are provided by a team of specialists: general nurses Sandra Rimkuvienė, Birutė Grigienė, Svetlana Metiolkina, Lidija Pozingienė, Eglė Butkutė, Audronė Garbuzienė, Regina Ulinskienė; Dovilė Jaugelienė; Inga Cikas; Ilona Grigonienė.

    • On 1 July 2022, an updated Ambulatory Care Home (AChH) procedure came into force, which will help to assess patients' needs more quickly and provide the right services in a timely manner. Until now, ASN services have been provided to patients who have been assessed by the Disability and Incapacity Determination Service as having a disability or special needs. Since 1 July, an individual questionnaire to determine the need for care has been introduced, which allows for a quicker conclusion and faster provision of nursing and care services. The new questionnaire helps to determine the level of care needs: low, medium or high. The number of services reimbursed by the Compulsory Health Insurance Fund depends on the patients' needs.

    • To increase access to services for patients living in rural areas who are unable to travel to the nearest medical facility, we are expanding the mobile medical unit. In the past, the mobile unit was limited to travelling every other week to certain rural areas to provide nursing services.

    • There is a growing need for outpatient nursing services in patients' homes in rural areas. Residents are seeking medical help themselves, so in cooperation with family doctors, the Mobile Medical Unit's car is used to visit patients in their homes. Nursing service professionals visit patients not only in the city but also in rural areas.

    • Outpatient nursing services provide:

    • Nurse - performs injections, IV insertion, care and catheter infusion under doctor's orders; collects blood, urine, etc. for laboratory tests; performs electrocardiogram, wound care, pressure ulcer prophylaxis and care, artificial orifice care (stoma care), ACS, pulse, etc.

    • Nurse assistant - performs pressure sore prevention, provides personal hygiene care (feeds or bathes the patient, nails, diaper changes, drops, oral care, etc.), handles the patient's secretions and excretions as assigned by the nurse (changes urine collectors, assists the patient to use the toilet chair, etc.), etc.

    • Physiotherapist - assesses and treats muscle, joint and nerve dysfunctions based on the patient's medical condition. Specifically, a physiotherapist can help to facilitate movement and function, reduce pain, and increase movement potential. A physiotherapist also teaches and helps to adapt and compensate for loss of function due to illness or injury.

    • Outpatient home nursing services are provided free of charge to patients who are registered with VšĮ Kėdainiai PSPC.

    • Home nursing services are prescribed by the patient's GP. He or she completes the "Extract from medical records" (form 027/a), which provides information on the patient's health status, justifies the indications for home care services, and writes recommendations and prescriptions to be followed by the nurse.

    • When a patient is being treated in hospital and the doctor foresees that he or she will need home nursing services when he or she returns home, he or she informs the Head of Nursing and provides an extract from the medical records (form 027/a) with prescriptions and recommendations.

    • With the doctor's note, the nurse arranges a time for the patient or his/her family to come in. At the first visit, the nursing staff assesses what services the patient needs and determines how these services will be provided. This shall be communicated to the patient and/or his/her relatives.

    • Community home nurses are in regular contact with the patient's doctor to help solve problems.

    • The nursing service is co-located with the GMP service.

    07. Diabetes nurse services

    Nursing services for adult patients with diabetes mellitus (DM) are provided at the Kėdainiai PSPC office No.3. The diabetes nurse provides initial and ongoing consultative care (information and counselling on nursing issues to prevent complications of CD, identify and help solve health and psychosocial problems of the patient and his/her family), diabetic foot care, and therapeutic pedicures.

    • Counselling support is provided in accordance with the CD training programme approved by the personal health care institution. The provision of CD care for adults is open to the patient's family if they wish.

    • These services are free of charge with a referral from a family doctor. Pre-registration is required. If the patient needs more services, paid services are available according to the Kėdainiai PSPC's fees for paid services.

    Diabetes mellitus (DM) patients receive nursing care with a referral from their family doctor to a diabetes nurse specialist.

    Free nursing services for people with diabetes

    - Changes in treatment (e.g. starting insulin treatment, changing insulin preparations, outpatient treatment with insulin pumps or introducing new high-tech tools, etc.);

    - women with CD during pregnancy.

    3. Therapeutic pedicure - hygienic, preventive and therapeutic pedicure;- removal of pressure sores, hyperkeratosis (squamous skin);

    - trimming normal and deformed nails;

    - grinding deformed nails with special equipment;

    - care of ingrown and fungal nails;

    - grinding of soles;

    - Applying a skin-softening cream to the feet;

    - Providing the patient and/or family with knowledge about foot care.

    Maximum of 4 services per year - Wagner grade 0 ulcer;- blindness;

    - patient age - over 65 years;

    - diabetic foot.

    4. Diabetic foot care - removal of hyperkeratosis;- necrectomy;

    - ulcer care;

    - Providing the patient and/or family with knowledge on foot and/or bigeye ulcer care.

    Maximum 24 services per year A patient with CD grade 0-3 according to Wagner has leg ulcers, ulceration and/or deep tissue caused by neurological or vascular changes in the lower extremities.

    For more than a decade, the Kėdainiai District Maternity School has been organising classes in which Kėdainiai PSPC specialists - obstetricians, gynaecologists, paediatricians, psychologists, physiotherapists, natural nutritionists and other specialists - have been trying to answer all the questions that parents-to-be may have about their children. The course provides information on the physiological changes that take place in a woman's body during pregnancy, describes the peculiarities of fetal development, explains how childbirth takes place, provides general information on newborn care and vaccinations, highlights the benefits of natural feeding and first tooth care. Although personal invitations to the programmes of the Maternity School are given to expectant mothers at the time of their doctor's appointment, expectant fathers are always welcome to attend the sessions.

    A wide range of educational/information materials are distributed to participants - leaflets, mother's books, natural feeding guides, brochures, guides for women who have given birth, pregnancy calendars, baby diaries, baby's first year books, early childhood education guides, and many more. Regular cooperation with various companies enables us to give parents-to-be the gift of Philips Avent, Johnson's baby, Colgate and other products. We also give recordings of Mozart's music, lullabies, the film "ABC of baby hygiene", and photos of the sessions.

    Motherhood is a big and important turning point in life, a step towards maturity and development. Expecting a baby, especially the first one, raises a lot of questions: what is recommended and what should be avoided during pregnancy, is childbirth really so scary, how to deal with the new baby, what and how to feed it... There are many different literature and press sources, but the data they provide are often contradictory. Advice from mothers and grandmothers in the vicinity, though lovingly and caringly expressed, may no longer be up-to-date or in line with medical developments. The courses are also attended by a number of women who will not be giving birth for the first time and who share their personal experiences and experiences.
    Pregnant women receive a lot of practical information when they visit the Obstetrics and Gynaecology Department. Here they can get to know the environment, equipment and staff in the maternity and postnatal wards. During the visit, they discuss birthing methods, anaesthetic options and other relevant topics. This session is included in the course programme.

    Expectant mothers are encouraged to start learning prenatal exercise as early as possible, as this gives the baby more oxygen and improves metabolism. Exercise also has a positive effect on the correct position of the foetus in the womb. During each session, participants have the opportunity to take part in practical exercises on "Pre/postnatal exercise", using special equipment ("Gymnic" balls, sticks, bands, etc.).

    The more mums and dads-to-be know about human development before birth, the better they can understand their baby's needs and feel confident and self-assured. Then the worries of giving birth, breastfeeding and raising a baby will not be frightening, but pleasant and joyful.
